Permission Number Request Form
Please complete this form to request permission numbers for your dance courses. Enter class titles in the "class" field (i.e., Modern 6), and enter either section number (i.e., 2) or instructor(s) name(s) (i.e., Paula and Mathew) in the "section" field. Click submit when you are finished. You will receive an e-mail with your permission numbers in 1-2 business days.
